The Nurse Practitioners and/or Physician Assistants in our Incidental Medical Services (IMS)-licensed Programs play a crucial role in ensuring quality care for our clients. The role evaluates medical appropriates for treatment, completes health screenings, conducts physicals, monitors the clients for any potential medical problems that would preclude a safe stay at the facility, ensures that withdrawal management medications including medication assisted treatment (MAT) are prescribed, while conducting patient rounding as needed to ensure safe stabilization. Additionally, this role in collaboration with the Medical Director will refer out for medical care when appropriate.

PRIMARY ACCOUNTABILITIES & RESPONSIBILITIES:

  • Service Delivery

    Responsibilities:  

    • Interprets vitals as collected by LVN and/or other healthcare assistant staff working within their scope of practice
  • Reviews history & physical information including communicable diseases from intake to inform quality clinical care while in treatment.  

  • Interprets SUD history coupled with drug test data to inform one’s safe stabilization in treatment    

  • Completes all documentation in an accurate and timely manner  

  • Depending on patient acuity, periodically observes residents for signs of mental or physical decompensation that may require a higher level of care , consulting with MD as needed  

  • Reviews withdrawal assessment scales (CIWA) -(COWS ), leveraging results in order to safety stabilize withdrawal symptoms through prescribing of withdrawal management medications   

  • Assists in collecting treatment appropriateness / fitness information for the Medical Director when needed  

  • Demonstrates an understanding of the pharmacology and physiological effects of alcohol and other commonly abused substances  

  • P rescribe withdrawal management medications including medication assisted treatment (MAT) under a doctor’s supervision, perform medical tests and record information about the current conditions of patient's health .  

  • Reports all Medical Incidents and/or unusual occurrences to Program Manager, Compliance Manager , and MD   

  • Works closely with other HSI staff and referral resources to provide quality patient care , communicating and coordinating as a member of a multi-disciplinary treatment team ( LVNs, Manager, MD, Clinical Supervisor, Counselors, etc.)  

  • Consistently inputs information in the computer, electronic health records, and other record-keeping systems to ensure treatment plans and progress notes are complete and up to date according to agency policy  

  • Makes or receives calls related to client care as needed  

  • Provides support if a deviation from medication orders is requested  

  • Supports the program operation by offering guidance & mentorship to other HCP staff   

  • Maintains health-related progress notes when conducting patient rounding.   

  • Develops and maintains positive working relationships with all staff members, Agency, and County Contracts.  

  • Maintains attendance and punctuality to perform duties as scheduled.  

  • Attends all staff meetings and required agency meetings unless excused.  

  • Maintains a clean environment, adhering to best practices   

  • Other duties as requested by HSI Medical Director and senior leadership team   

  • Ensures to Review LVN/LTP documentation and builds LVN and RN understanding of IMS services .

    QUALIFICATIONS:    (education and work experience)

  • NP PA License for state of California 

  • Not required but preferred 6 month to 1 year of substance use disorder, mental health, or similar. 

  • CPR current or able to obtain first month of hire.

    Salary:   $140K-$150K/ Annually depending on experience and qualifications. Available for a full-time position.
